Grid bugging out

As a healer I love using Grid but I have this issue with it getting stuck and not updating the hp of people in the raid. Meaning that grid is telling me someone is half health when they are full and vice versa. Does this happen to anyone else or is my version just corrupted? If so does anyone have a fix for this issue?
